مقدمه: نسبت پروژسترون/ اووسیت متافاز دو به نظر می رسد شاخص بالینی مفیدی جهت نشان دادن میزان لانه گزینی در لقاح خارج رحمی باشد.
هدف: به منظور تعیین اینکه آیا افزایش پروژسترون سرم در روز تجویز HCG کمکی به شناسایی چگونگی قدرت پذیرش آندومتر می­کند؟
مواد و روش­ها: در یک مطالعه مقطعی در مرکز تحقیقاتی و درمانی ناباروری یزد، 378 سیکل IVF / ET از اکتبر 2009 تا مارس 2011 بررسی شد. میزان استروژن ( پیکو گرم در میلی لیتر) و پروژسترون (نانوگرم در میلی لیتر) در روز تجویز hCG اندازه گیری شد. نسبت پروژسترون/ استروژن و نسبت پروژسترون/تخمک بالغ محاسبه شد. تأثیر این نسبت­های هورمونی بر روی میزان لقاح ، میزان لانه گزینی و میزان حاملگی ارزیابی شد.
نتایج: در این مطالعه ، مقدار مطلق پروژسترون و نسبت پروژسترون/ استروژن پیش­بینی­کننده خوبی برای نتایج  لقاح آزمایشگاهی نبودند. نسبت پروژسترون/ تخمک بالغ پیش­بینی­کننده میزان لانه گزینی و میزان بارداری با نتایج قابل توجه آماری بود اما هیچ تأثیری بر میزان لقاح نداشت.
نتیجه­گیری: ما پیشنهاد می­کنیم برای جلوگیری از افزایش پروژسترون که علت بلوغ پیشرفته آندومتر و اختلال در پذیرش اندومتر می­باشد اقدامات زیر انجام پذیرد. دوز کم هورمون hCG به تنهایی در مراحل  انتهائی COH، استفاده از درمان با آگونیست با قدرت کمتر ( کمتر تهاجمی)،  پروتکل آنتاگونیست انعطاف پذیر و دوز کم هورمون hCG به تنهائی همزمان با شروع آنتاگونیست، اضافه کردن  آنتاگونیست GnRH با پروتکل microdose، Mifepristone، اگر سطح پروژسترون سرم بیش از 32/0به ازای هر تخمک متافاز ІІ باشد بهتر است انتقال جنین لغو شده و انجماد همه جنین­ها برای انتقال در آینده که آندومتر بیشتر پذیرا باشد در نظر گرفته شود.
