en تشخیص حاملگی خارج از رحمی بینابینی با استفاده از سونوگرافی سه بعدی و مدیریت لاپاروسکوپیک آن: گزارش یک مورد Interstitial ectopic pregnancy diagnosis by three-dimensional ultrasound and its laparoscopic management: A case report Assisted Reproductive Technologies Case Report Case Report مقدمه: بارداری خارج رحمی بینابینی (IEP) یک نوع نادر از بارداری خارج رحمی است که با خطر پارگی و خونریزی مواجه است. بروز IEP حدود 2-4% از کل بارداریهای خارج رحمی است که تشخیص و مدیریت آن چالش برانگیز است. ما یک مورد از IEP را که تشخیص و مدیریت به موقع آن انجام شد را گزارش کردهایم. مورد: یک خانم 37 ساله سه هفته پس از آزمایش بارداری مثبت، با شکایت درد ناگهانی لگن و خونریزی واژینال، در مؤسسه رویان پذیرش شد. او با روشIVF  باردار شده بود. او جهت سونوگرافی سهبعدی پذیرش شد و یک ساک بارداری (GS) با جنین زنده و کیسه زرده در فوندوس و خارج از مرکز آندومتری دیده شد. احتمال بارداری خارج رحمی مطرح شد. تایید تشخیص بیشتر توسط سونوگرافی سهبعدی انجام شد و درمان توسط لاپاراسکوپی انجام شد. بیمار تحت عمل جراحی رزکسیون کورنئال چپ قرار گرفت. او بعد دو روز از افت کامل β -HCG (کمتر از 5 mIU/mL) مرخص شد و سایر پیگیریهای روتین انجام شد. فرم رضایتنامه از بیمار برای گزارش مورد تکمیل شد. نتیجه گیری: با توجه به عوارض خطرساز و تهدیدکنندهی حیات مرتبط با IEP، آشنایی و شک به موارد محتمل بارداری خارج رحمی بینابینی اهمیت دارد. اطلاعات جزئی که توسط سونوگرافی سهبعدی به دست میآید, میتواند برای تشخیص صحیح و محل دقیق آن مفید باشد. Background: Interstitial Ectopic Pregnancy (IEP) is an uncommon type of ectopic pregnancy with the risk of rupturing and bleeding. The incidence of IEP is about 2-4% of all EPs. The diagnosis and management are challenging. We present a well-timed and managed case of IEP. Case: The case was a 37-yr-old woman presented at the Royan Institute with a chief complain of sudden onset of pelvic pain and moderate vaginal bleeding, three weeks after her positive pregnancy test. She had got pregnant with in-vitro fertilization procedure. She was admitted for a two-dimensional ultrasound (2DUS). The 2DUS findings showed a gestational sac with live embryo and yolk sac which was located high in the fundus and eccentric to the endometrium. The suspicion of IEP rose after the 2DUS findings, the confirmation of further diagnosis was then done by three-dimensional ultrasound, and the treatment was done by laparoscopy. The patient underwent laparoscopic left corneal resection. She was discharged after two days and her β-hCG achieved complete resolution (< 5 mIU/mL) after two weeks’ follow-up. Conclusion: According to the life-threatening complications that are associated with IEP, acquaintance and suspicion about IEP is important.
