en تعیین جنسیت و ویژگی های روانی-جنسی افراد مبتلا به اشکال مختلف سندرم عدم حساسیت به آندروژن: یک مطالعه کیفی Sex assignment and psychosexual peculiarities of individuals with different forms of androgen insensitivity syndrome: A qualitative study Reproductive Genetics Original Article Original Article <div style="text-align: justify;">مقدمه: عدم تطابق بین جنسیت‌های کروموزومی، هورمونی و فنوتیپی در افراد مبتلا به سندرم عدم حساسیت آندروژنی (AIS) مشکلاتی را در تعیین جنسیت و شناسایی روانی‌ جنسی ایجاد می‌کند. هدف: هدف از این مطالعه شناسایی ویژگیهای روانی جنسی و تعیین جنسیت افراد مبتلا به اشکال مختلف AIS بود. مواد و روشها: در این مطالعه کیفی، 41 فرد بین 15 تا 31 سال مبتلا به AIS که بین سال‌های 2016 تا 2021 به مرکز پزشکی تولیدمثل یونیورس تفلیس، گرجستان مراجعه کرده بودند، مورد بررسی قرار گرفتند. همه افراد تحت معاینات بالینی، جنسی، هورمونی، سونوگرافی و سیتوژنتیک قرار گرفتند. مصاحبههای کامل و سوابق پزشکی جهت بررسی پروفایلهای روانی-جنسی و سابقه تعیین جنسیت مورد ارزیابی قرار گرفت. نتایج: 32 نفر با فرم کاملAIS  (CAIS)، 8 نفر با فرم جزئی (PAIS) و یک نفر با فرم خفیف (MAIS) تشخیص داده شدند. افراد مبتلا به CAIS و PAIS در بدو تولد مورد ارزیابی قرار گرفته و به عنوان دختر بزرگ شده بودند. افراد مبتلا به CAIS و تمایل روانی-جنسی زنانه به دلیل آمنوره به ما ارجاع داده شده بودند. نوجوانان مبتلا به PAIS که به عنوان دختر بزرگ شده بودند به دلیل عضلانی شدن مردانه در بلوغ به ما مراجعه کرده بودند. یک فرد مبتلا به MAIS که در بدو تولد به عنوان پسر مورد ارزیابی قرار گرفته و بزرگ شده بود اندام تناسلی مردانه داشت. همه افراد مبتلا به AIS داده‌های هورمونی معمولی و کمپلکس کروموزومی جنسی برای مردان داشتند. 20 فرد فعال جنسی مبتلا به CAIS با مردان تماس جنسی آلت تناسلی و واژن داشتند. هیچ یک از افراد مبتلا به CAIS و PAIS پس از تشخیص، به تغییر جنسیت فکر نکردند، و فقط فرد مبتلا به MAIS تصمیم به تغییر جنسیت از مرد به زن گرفت. نتیجهگیری: شناسایی روانی-جنسی یک چالش مهم در مدیریت AIS است. تشخیص تغییرات روانی-جنسی در یک زن شرکتکننده در مطالعه که برای MAIS غیرمعمول است، ممکن است با موزاییسم سوماتیک ژن گیرنده آندروژن همراه باشد.</div> <div style="text-align: justify;">Background: A mismatch between chromosomal, gonadal, and phenotypic sexes in individuals with androgen insensitivity syndrome (AIS) creates problems in sex assignment and psychosexual identification. Objective: To identify psychosexual and sex assignment peculiarities of individuals with different forms of AIS. Materials and Methods: In this qualitative study, 41 individuals with AIS aged between15-31 yr <a name="_Hlk130810769">who referred to the Universe Center for Reproductive Medicine Tbilisi, Georgia between 2016 and </a>2021 were studied. All individuals underwent clinical, genealogical, hormonal, ultrasonographic, and cytogenetic examinations. In-depth interviews and medical records <a name="_Hlk130810859">assessed psychosexual profiles and sex assignment histories. </a> Results: 32 cases were diagnosed with the complete form of AIS (CAIS), 8 individuals with the partial form (PAIS), and one with a mild form (MAIS). Individuals with CAIS and PAIS were assessed at birth and raised as girls. Individuals with CAIS and female psychosexual disposition were referred to us due to amenorrhea. Adolescent individuals with PAIS assessed as girls referred to us due to masculinization detected in puberty. An individual with MAIS was assessed at birth and raised as a boy with male genitalia. All individuals with AIS had typical hormonal data and sex chromosome complex for men. 20 sexually active individuals with CAIS had penile-vaginal contact with the man. None of the individuals with CAIS and PAIS thought about gender reassignment after being diagnosed, only the individual with MAIS aimed for male-to-female transition. Conclusion: Psychosexual identification remains a significant challenge in AIS management.
