en تورشن فیمبریا به عنوان یک علت نادر درد شکم: گزارش یک مورد و مرور متون Reproductive Surgery Case Report Case Report <div style="text-align: justify;"><span style="font-size:12pt"><span style="text-justify:inter-ideograph"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b><span lang="FA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:">مقدمه:</span></span></b><span lang="FA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:"> تورشن فیمبریا یک اورژانس نادر در حوزه بیماری‌های زنان است که به دلیل علائم غیراختصاصی مانند درد در ربع تحتانی راست شکم و تهوع، یک چالش تشخیصی مهم محسوب می‌شود. این وضعیت می‌تواند به راحتی با بیماری‌های شایع‌تری مانند آپاندیسیت حاد اشتباه گرفته شود. تشخیص به موقع برای جلوگیری از عوارض جدی مانند نکروز بافتی ضروری است.</span></span></span></span></span></span></span><br> <span style="font-size:12pt"><span style="text-justify:inter-ideograph"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b><span lang="FA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:">مورد:</span></span></b><span lang="FA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:"> خانم ۴۷ ساله با درد پیشرونده در ربع تحتانی راست شکم و تهوع به اورژانس مراجعه کرد. سونوگرافی شکم، تشخیص احتمالی آپاندیسیت حاد را مطرح کرد. بیمار تحت لاپاراسکوپی تشخیصی قرار گرفت. در حین جراحی، آپاندیس تنها التهاب خفیفی داشت، اما تورشن ایزوله و گانگرنوز فیمبریای لوله فالوپ راست کشف و بافت مربوطه برداشته شد.</span></span></span></span></span></span></span><br> <span style="font-size:12pt"><span style="text-justify:inter-ideograph"><span style="direction:rtl"><span style="unicode-bidi:embed"><span new="" roman="" style="font-family:" times=""><b><span lang="FA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:">نتیجه&shy; گیری:</span></span></b><span lang="FA" style="font-size:11.0pt"><span b="" mitra="" style="font-family:"> این مورد، اهمیت در نظر گرفتن تورشن فیمبریا در تشخیص افتراقی درد ربع تحتانی راست شکم در زنان را برجسته می‌کند، حتی زمانی که علائم بالینی نشان‌دهنده علت شایع‌تری باشد. ارزیابی جامع حین عمل زمانی که یافته‌های اولیه به طور کامل تظاهرات بالینی را توضیح نمی‌دهند، بسیار مهم است و مداخله به موقع و پیشگیری از عوارض طولانی مدت را تضمین می‌کند.</span></span><span dir="LTR" style="font-size:11.0pt"><span style="font-family:"Arial",sans-serif"></span></span></span></span></span></span></span></div> <div style="text-align: justify;"><span style="font-size:14px;"><span style="font-family:Times New Roman;"><span style="line-height:normal"><b><span new="" roman="" times="">Background: </span></b><span new="" roman="" times="">Isolated fimbrial torsion is a rare gynecological emergency that can lead to severe complications if not promptly diagnosed. Its clinical presentation often mimics more common causes of abdominal pain, creating a significant diagnostic challenge.</span></span><br> <span style="line-height:normal"><b><span new="" roman="" times="">Case Presentation: </span></b><span new="" roman="" times="">This case report describes a 47-yr-old woman presenting with right lower quadrant abdominal pain. The initial clinical and ultrasound findings were suggestive of acute appendicitis. However, diagnostic laparoscopy revealed isolated gangrenous torsion of the right fallopian tube fimbria.</span></span><br> <span style="line-height:normal"><b><span new="" roman="" times="">Conclusion: </span></b><span new="" roman="" times="">This case underscores the importance of including fimbrial torsion in the differential diagnosis of right lower quadrant pain in women, even when clinical signs suggest a more common etiology.
