en ارزش تشخیصی اندازه گیری آنزیم های AST و ALT در مایع واژینال در حاملگی زودرس Predictive value of aspartate aminotransferase and alanine aminotransferase levels in vaginal fluid for the diagnosis of premature rupture of membranes Original Article Original Article مقدمه: پارگی زودرس کیسه آب از زمان قبل از ترم (PPROM) در 3% حاملگیها روی میدهد و 40-30% زایمانهای قبل از ترم را شامل میشود. به علت عوارض متعددی که پارگی طولانی کیسه آب و زایمان پرهترم روی جنین دارد تشخیص زودرس آن امری ضروری به نظر میرسد هدف: ین مطالعه به منظور اندازه گیری AST و ALT در مایع واژینال برای تشخیص پارگی زودرس کیسه آب در زمان قبل از ترم بعنوان یک تست با ارزش و غیر تهاجمی انجام شد. مواد و روشها: بررسی حاضر مطالعهای مقطعی با گروه کنترل است که روی 148 نفر خانم باردار انجام شده است افراد به دو گروه 74 نفره یک گروه خانمهای 36-26 هفته با کیسه آب سالم (شاهد) و یک گروه خانمهای 36-26 هفته با کیسه آب پاره (مورد) تقسیم شدند. اندازه گیری AST و ALT در مایع واژینال در هر دو گروه انجام شد. برای مقایسه میانگین AST و ALT از آزمون mann- whitney u test استفاده شد. نتایج: میانگین AST در مایع شستشوی واژینال در افراد گروه مورد Iu/Lit  12/77±10/06 و در افراد کنترل Iu/Lit 6/91±10/92 بود که تفاوت آماری معنیداری داشت (0/001>p). همچنین در این بررسیها هیچ تفاوت معنیداری در میزان ALT بین دو گروه مشاهده نشد (0/49=p).  نقطه برش برای AST در تشخیص PPROM، Iu/L 5/4 بود. حساسیت آن 82/47، ویژگی 63/5%، ارزش پیش گویی کننده مثبت و منفی بترتیب 69/32 و 78/33 بود. نتیجهگیری: براساس یافته های این مطالعه اندازه گیری AST در مایع واژینال می تواند بعنوان یک تست قابل اطمینان برای تشخیص PPROM بکار رود. اما در این مور نقطه برش خوبی برای ALT وجود ندارد. Background: Preterm premature rupture of membranes (PPROM) occurs in 3% of pregnancies and 30-40% of preterm labors are related to this problem. Early diagnosis of PPROM is very important due to its impact on pregnancy outcomes. Objective: To determine the diagnostic value of aspartate aminotransferase (AST) and alanine aminotransferase (ALT) levels in vaginal fluid for the diagnosis of preterm premature rupture of membranes as a non-invasive and available test. Materials and Methods:  A total of 148 pregnant women between the 26th-36th gestational weeks were enrolled in the study. 74 patients were in PROM group and 74 in control group. AST and ALT levels in vaginal fluid were measured in each group. Mann Whitney U-test was used to compare AST and ALT levels in each group. Results:  The mean of AST level in vaginal fluid was 12.77±10.06 in PROM group vs. 6.91±10.92 in control group (p<0.001), while there were no significant difference between ALT levels in PROM group 1.51±3.17 and control group 0.89±1.15 (p=0.49). Optimal cut point of AST for the diagnosis of PROM was 4.5 IU/L in this study. The sensitivity, specificity, positive and negative predictive values were 82.4%, 63.5%, 69.32% and 78.33% respectively. Conclusion:  According to the findings of this study, measurement of AST level in vaginal fluid can be used as a reliable test for diagnosis of PROM, but there is no good cut point for ALT level that can be practically used. آسپارتات آمینو ترانس فراز, آلانین ترانس آمیناز, پارگی زودرس پرده های جنینی, زایمان زودرس.
