پروتکل توقف آگونیست GnRH نسبت به پروتکل آنتاگونیست در نتایج تکنیکهای کمکباروری در پاسخدهندگان ضعیف برتری ندارد: یک کارآزمایی بالینی تصادفی. International Journal of Reproductive BioMedicine. 1405; 24 (7) :627-636

مقدمه: پاسخ ضعیف تخمدان همچنان یکی از چالشبرانگیزترین مشکلات در فناوریهای کمکباروری (ART) به شمار میرود. برای بهبود نتایج درمان، پروتکلهای مختلف تحریک تخمدان پیشنهاد شدهاند که از جمله آنها میتوان به راهبردهای ترکیبی شامل هورمون آزادکننده گنادوتروپین (GnRH) آگونیست و آنتاگونیست اشاره کرد.
هدف: مقایسه اثربخشی پروتکل توقف آگونیست GnRH همراه با پروتکل آنتاگونیست GnRH در مقایسه با پروتکل استاندارد آنتاگونیست GnRH در بیماران با پاسخ ضعیف تخمدانی.
مواد و روش ها: این کارآزمایی بالینی تصادفی بر روی ۱۰۰ زن 44-18 ساله که بر اساس معیارهای بولونیا به عنوان پاسخدهندگان ضعیف تخمدانی تشخیص داده شده بودند، انجام شد. شرکتکنندگان بهطور تصادفی به 2 گروه تقسیم شدند: گروه کنترل که پروتکل آنتاگونیست GnRH دریافت کردند و گروه مداخله که پروتکل ترکیبی توقف آگونیست GnRH/آنتاگونیست GnRH دریافت کردند. پیامد اصلی، نرخ بارداری بالینی بود. پیامدهای ثانویه شامل تعداد اووسیتهای بازیابیشده، کیفیت جنین، بارداری شیمیایی و نرخ تولد زنده بودند.
نتایج: از نظر میزان بارداری شیمیایی، بارداری بالینی و تولد زنده، تفاوت آماری معناداری بین 2 گروه مشاهده نشد. همچنین پیامدهای آزمایشگاهی شامل تعداد اووسیتهای بازیابیشده، تعداد اووسیتهای متافاز II و کیفیت جنین در 2 گروه مشابه بود. با این حال، گروه مداخله به دوز بالاتری از گنادوتروپین نیاز داشتند و مدت زمان تحریک تخمدان در آنها طولانیتر بود (01/0 > p).
نتیجه گیری: پروتکل توقف آگونیست GnRH در بیماران با پاسخ ضعیف تخمدانی از نظر پیامدهای آزمایشگاهی یا بالینی نسبت به پروتکل آنتاگونیست GnRH برتری نشان نداد.
