مقدمه: آزواسپرمی غیرانسدادی (NOA) یکی از شدیدترین اشکال ناباروری مردان است که با اختلال در اسپرماتوژنز مشخص می‌شود. اگرچه ناهنجاری‌های کروموزومی و حذف‌های میکروسکوپی کروموزوم Y از علل شناخته‌شده این بیماری هستند، اما بسیاری از موارد همچنان ایدیوپاتیک باقی می‌مانند که نشان‌دهنده نقش احتمالی عوامل ژنتیکی دیگر است. ژنRAD54 like  (RAD54L) یکی از اجزای کلیدی مسیر نوترکیبی همولوگ است که در ترمیم شکست‌های دو رشته‌ای DNA و پیشرفت میوز نقش دارد.
هدف: هدف از مطالعه حاضر شناسایی علل ژنتیکی احتمالی NOA با استفاده از توالی‌یابی کل ژنوم  در افراد مبتلا از خانواده‌های خویشاوند است.
مواد و روش ­ها: در این مطالعه که در سال 2023 انجام شد، 10 مرد مبتلا به NOA که به مرکز ناباروری پژوهشکده علوم تولیدمثل یزد ارجاع شده بودند، تحت ارزیابی‌های بالینی، هورمونی و ژنتیکی قرار گرفتند. DNA ژنومی از خون محیطی استخراج و برای WGS مورد استفاده قرار گرفت. واریانت‌ها بر اساس فراوانی آللی، پیش‌بینی بیماری‌زایی، الگوی توارث و داده‌های بیان ژنی فیلتر و اولویت‌بندی شدند. تحلیل‌های بیوانفورماتیکی اسپلایسینگ با استفاده از SpliceAI و Human Splicing Finder انجام شد.
نتایج: یک واریانت اسپلایس‌سایت هموزیگوت بسیار نادر c.1375+1G>T در ژن RAD54L شناسایی شد. این واریانت در محل donor splice site قرار داشت و پیش‌بینی شد موجب اختلال در اسپلیسینگ طبیعی شود که احتمالاً منجر به حذف اگزون 12، ایجاد فریم‌شیفت و تشکیل کدون پایان زودرس می‌گردد. ژن RAD54L بیشترین بیان را در بافت بیضه دارد و نقش مهمی در نوترکیبی همولوگ و ترمیم DNA در میوز ایفا می‌کند. بیمار دارای یافته‌های هورمونی سازگار با نارسایی اولیه بیضه بود.
نتیجه­ گیری: این مطالعه ژن RAD54L را به‌عنوان یک کاندید ژنی بالقوه جدید مرتبط با NOA مطرح می‌کند و بر کارایی WGS در شناسایی علل ژنتیکی نادر ناباروری مردان ایدیوپاتیک تأکید دارد. مطالعات عملکردی و بررسی‌های بالینی بیشتر برای تأیید این ارتباط ضروری است.
