مقدمه: DNA آزاد سلولی (cfDNA) موجود در محیط کشت جنین (SCM) گزینه‌ای غیرتهاجمی برای تست ژنتیکی پیش از لانه‌گزینی است؛ با این حال، غلظت پایین و تکه‌تکه‌شدن این DNA، قابلیت اطمینان بالینی را محدود می‌سازد. تلفیق ارزیابی ژنتیکی با پروفایل متابولومیک می‌تواند اطلاعات مکملی را در مورد توانمندی رشدی جنین فراهم کند.
هدف: در این مطالعه هدف بهینه‌سازی استفاده از cfDNA در محیط کشت بلاستوسیست با استفاده از روش‌های پیش‌ آماده‌سازی و همچنین پروفایل اسید آمینه به عنوان یک شاخص مکمل برای تشخیص سلامت جنین بوده است.
مواد و روش­ها: در این مطالعه مورد-شاهدی (1401-1399) 90 جنین با روش هیبریداسیون فلورسنس در جا (FISH) و یا هیبریداسیون مقایسه‌ای ژنومی (array CGH) شناسایی و بررسی شدند. محیط کشت‌ جنین‌های منتخب به 3 گروه (30 = n) پیش تکثیر شامل: جوشاندن سریع، خالص‌سازی مبتنی بر سیلیکا و تکثیر کل ژنوم (WGA) تقسیم شد. تعیین جنسیت با روش واکنش زنجیره‌ای پلیمراز کمی (qPCR) انجام شد. برای کنترل کیفی cfDNA و غربالگری آنیوپلوئیدی، از کیت multiplex IRFiling و واکنش زنجیره‌ای پلیمراز فلورسنت کمی (QF-PCR) استفاده گردید. پروفایل آمینواسیدی در مراحل مختلف رشد جنینی و درجات کیفی متفاوت، از طریق کروماتوگرافی مایع-طیف‌سنجی جرمی (LC-MS/MS) کمی‌سازی شد.
نتایج: جوشاندن سریع منجر به شکست کامل در تقویت DNA شد. در مقابل، خالص‌سازی مبتنی بر سیلیکا، تطابق 70% برای تعیین جنسیت با qPCR و 7/%56 برای QF-PCR نشان داد. روش WGA به بالاترین کارایی دست یافت (3/73% برای qPCR و 7/56% برای QF-PCR)، اگرچه بررسی‌های کنترل کیفیت، مواردی از طبقه‌بندی اشتباه را گزارش کرد. در مورد پروفایل اسید آمینه با استفاده از LC-MS/MS آلانین و آرژنین به‌طور معناداری سطوح بالاتر را در جنین‌های تری‌پرونوکلئار (3PN) نشان داد. علاوه بر این، در بلاستوسیست‌های باکیفیت سطوح بالاتری از اسید گلوتامیک همراه با کاهش کلی آمینواسیدهای خارج‌ سلولی در مقایسه با جنین‌های باکیفیت پایین و گروه‌های کنترل نمایان بود.
نتیجه­گیری: روش WGA در مقایسه با دو روش دیگر، توانست، شناسایی و تشخیص cfDNA را در محیط کشت جنین بهبود بخشد؛ اما با شرایط بررسی حاضر برای niPGT بالینی به تنهایی کافی نیست و نیاز به توسعه و بهبود شرایط دارد. در مورد پروفایل اسیدآمینه نیز این روش می­تواند به عنوان مکمل برای نشان دادن وضعیت کروموزومی و پتانسیل رشد جنین در یک استراتژی چند وجهی برای ارزیابی غیرتهاجمی جنین در PGT به کارگرفته شود.
