ارزیابی اثر اینترالیپید بر پیامدهای بارداری در زنان با سابقه شکست مکرر لانه‌گزینی: یک مطالعه کوهورت گذشته‌نگر

مقدمه: شکست مکرر لانه‌گزینی (RIF) همچنان یکی از چالش‌ها در فناوری‌های کمک‌باروری به شمار می­رود. اینترالیپید به‌عنوان یک مداخله تعدیل‌کننده ایمنی برای بهبود پیامدها در افراد با سابقه RIF پیشنهاد شده است، هرچند شواهد موجود در این زمینه همچنان متناقض می­باشد.
هدف: هدف این مطالعه ارزیابی اثر اینترالیپید بر پیامدهای بارداری در زنان با سابقه حداقل 2 سیکل انتقال جنین ناموفق بود.
مواد و روش ­ها: در این مطالعه کوهورت گذشته‌نگر، داده­ های پرونده پزشکی ۲۷۰ خانم با سابقه حداقل 2 بار انتقال جنین ناموفق قبلی، که مجدداً در طول سال ۱۴۰۰ خورشیدی در مرکز درمان ابن­سینا تحت سیکل انتقال جنین فریز قرار گرفته بودند، استخراج گردید، این افراد به 2 گروه تقسیم شدند: گروه اینترالیپید (۱۳۲ نفر) که ۱۰۰ میلی‌لیتر انفوزیون وریدی اینترالیپید 20% یک هفته قبل از انتقال جنین فریز شده (FET) دریافت کردند و در صورت مثبت بودن آزمایش بارداری، اینترالیپید با همان دوز قبلی هر 4 هفته تا پایان 3 ماهه اول بارداری تجویز می­گردید، و گروه کنترل (۱۳۸ نفر) که در سیکل‌های FET درمان‌های روتین را دریافت کردند. پیامدهای مورد بررسی شامل بارداری بیوشیمیایی، بارداری بالینی، بارداری در حال تداوم، سقط و تولد زنده بود. داده‌ها با استفاده از آزمون کای‌دو یا آزمون دقیق فیشر در نرم‌افزار SPSS تحلیل شدند.
نتایج: میزان بارداری بیوشیمیایی در گروه اینترالیپید به‌طور معنی‌داری بیشتر بود (5/51% در مقابل 7/37%، 022/0 = p). بارداری بالینی (2/43% در مقابل 2/%36)، بارداری در حال تداوم (8/28% در مقابل 8/18%) و تولد زنده (8/25% در مقابل 4/17%) نیز در گروه اینترالیپید بهبود نشان دادند، اگرچه این اختلاف‌ها از نظر آماری معنی‌دار نبودند (۰٫۰۵ < p). میزان سقط بین 2 گروه تفاوت معنی‌داری نداشت (8/25% در مقابل 0/21%، 357/0 = p).
نتیجه ­گیری: در خانم ­های با سابقه RIF تجویز اینترالیپید شاید میزان بارداری بیوشیمیایی را افزایش دهد و همچنین روند مثبتی ـ هرچند غیرمعنی‌دار ـ در سایر پیامدهای بارداری نشان می‌دهد. برای تأیید این یافته­ ها انجام کارآزمایی‌های بالینی تصادفی بزرگ‌ مقیاس ضروری می ­باشد.
